2. Certified copy of your child’s latest school report
3. Official proof of residence, e.g. most recent Municipal Water and lights account
4. A certified copy of your medical aid card
5. ID documents of parent (if you are the guardian of this child, it is necessary for official legal proof)
6. A photo (ID or Passport Photo)
